ECMO是一項高風險、高創傷、高消耗的創新技術，它能夠為新冠病毒肺炎重症患者提供挽救性治療°ECMO的治療目標是幫助患者恢復心肺功能，或者橋接最終治療，包括器械植入，或者器官移植等。然而，容易被忽視的 事實是，ECMO挽救了一些患者的生命，但也可能讓那些沒有康復機會的患者陷入醫療困境。於是，ECMO的臨床應用不得不面對一些反對意見，包括嚴重併發症危害患者生命安全、無效治療導致技術失敗，以及大量佔用資 源損害醫療公平等。ECMO技術的臨床應用應該在尊重生命價值和患者意願的基礎上，合理設置治療目標、確立可接受退出標準、妥善處理患者意願與ECMO設備撤除困境之間的倫理衝突，建立適度倫理框架以合理控制醫療 干預的邊界。 Characterized by high risk, high trauma and high consumption, Extra-Corporeal Membrane Oxygenation (ECMO) is an innovative technology that can be used as salvage therapy for COVID-19 patients. ECMO treatment can help restore patients' cardiopulmonary function or can bridge their final treatment, including device implantation or organ transplantation. However, although ECMO saves some patients' lives, it can also leave those with no chance of recovery in a medical dilemma. ECMO (...) is thus controversial: it is criticized for technical failures and ineffective treatments, and its use raises questions about medical equity. This paper argues that the clinical application of ECMO technology should be based on a respect for the value of life and the will of patients, reasonably set therapeutic goals, acceptable withdrawal criteria, proper management of the conflict between patients' desires and the medical dilemma, and the establishment of an ethical framework to control the limits of medical intervention. Viewing workplace deviance within a victim precipitation framework, we explore how abusive supervisors target subordinates low in core self-evaluations to explain when such employees respond by engaging in workplace deviance. We theorize that employees who are lower in CSE receive more abusive supervision, which generates subsequent harmful reactions toward supervisors, peers, and the organization. This occurs primarily when employees lack sufficient cognitive resources in dealing with supervisor abuse. We test, replicate, and extend our theoretical model in three empirical studies. Results (...) demonstrate that lower employee CSE drew more abusive supervision and led low-CSE employees to exhibit workplace deviance. This abusive supervision mediation effect was stronger for employees with comparatively lower cognitive ability levels. The findings are discussed with regard to theoretical and ethical issues in confronting employee abuse. (shrink)

It remains unclear whether such genetic variants are associated with disease aggressiveness. The NCI-SPORE Genetics Working Group retrospectively collected clinicopathologic information and genotype data for 36 SNPs which at the time had been validated to be associated with PC risk from 25,674 cases with PC. Cases were grouped according to race, Gleason score and aggressiveness. Statistical analyses were used to compare the frequency (...) of the SNPs between different disease cohorts. After adjusting for multiple testing, only PC-risk SNP rs2735839 was significantly and inversely associated with aggressive and high-grade disease in European men. Similar associations with aggressive and high-grade disease were documented in African-American subjects. The G allele of rs2735839 was associated with disease aggressiveness even at low PSA levels in both European and African-American men. Our results provide further support that a PC-risk SNP rs2735839 near the KLK3 gene on chromosome 19q13 may be associated with aggressive and high-grade PC. Future prospectively designed, case-case GWAS are needed to identify additional SNPs associated with PC aggressiveness. (shrink)

“The ineffable” in Wittgenstein’s Tractatus Logico-Philosophicus is an essential term that has various interpretations. It could be divided into two types, namely, positive and negative, or real and fake. The negative or fake type can be clarified by logical analysis, while the positive or real type can be understood only through philosophical critique. Both the positive and negative types consist of infinity or absoluteness, but the infinity is subject to distinctions in meaning and logic.

Dans le Philèbe de Platon, Socrate fait valoir que la vie bonne doit consister en la connaissance et le plaisir. Une partie de cette démonstration consiste en une analyse des parties de la connaissance où la connaissance peut être plus ou moins pure, plus ou moins mêlée d’éléments étrangers tels que la sensation ou l’expérience. Lorsqu’elle est pure, elle s’attache à la vérité, pure et simple. Car, nous devons l’admettre, la connaissance est vraie, quoiqu’elle puisse être d’autre par ailleurs. La (...) connaissance peut rendre les humains bons, c’est-à-dire leur permettre de bien faire, de manière fiable et souple, mais elle a son bien au-delà de l’existence humaine – elle n’est pas limitée à l’existence humaine. Nous suggérons ici que Platon détermine en fait la connaissance à partir de sa finalité – sa cause finale est utilisée pour déterminer sa cause formelle : ce que vise la connaissance nous dit ce qu’elle est. Au lieu de nous fournir une analyse selon l’aporie finale et insatisfaisante du Théétète (« l’opinion vraie accompagnée de raison »), la connaissance est ainsi dotée d’une justification fonctionnelle. Une partie de cette proposition est que la connaissance se doit d’être vraie ; c’est ce qui garantit sa stabilité : elle est à elle-même sa propre fin. Le bien de la connaissance, c’est la vérité. C’est ce qui lui permet de se comporter comme un guide pour l’âme. Les attributs additionnels de ce clan, qui est celui de la connaissance, découlent de la vérité et de leur relation à elle : exactitude, clarté et pureté. Car dans la mesure où ils sont purs et sans mélange avec des éléments étrangers tels que la pratique ou la sensation, ils ne s’intéressent qu’à des unités exactes, de choses qui sont ce qu’elles sont, de sorte que leur clarté n’est en rien troublée. The Greenberger-Horne-Zeilinger state is the most famous example of a state with multiparticle entanglement. In this article we describe a group theoretic framework we have been developing for understanding the entanglement in general states of two or more quantum particles. As far as entanglement is concerned, two states of n spin-1/2 particles are equivalent if they are on the same orbit of the group of local rotations (U(2)n). We consider both pure and mixed states and calculate the number of independent (...) parameters needed to describe such states up to this equivalence. We describe how the entanglement of states in a given equivalence class may be characterized by the stability group of the action of the group of local rotations on any of the states in the class. We also show how to calculate invariants under the group of local actions for both pure and mixed states. In the case of mixed states we are able to explicitly exhibit sets of invariants which allow one to determine whether two generic mixed states are equivalent up to local unitary transformations. Les trente dernières années du XX e siècle ont été marquées par une très forte inflation des règles du droit du travail visant à protéger l’emploi. Ces règles encadrent notamment le licenciement et tentent d’endiguer le recours aux contrats précaires. Mais on s’aperçoit aujourd’hui qu’il n’existe pas de rapport mécanique entre les objectifs poursuivis (protéger l’emploi), leur traduction législative et les résultats produits qui sont décevants. Prenant acte de son impuissance le législateur entend désormais changer de paradigme en concentrant ses (...) efforts sur la protection des transitions professionnelles. Toutefois les lois les plus récentes qui s’inscrivent dans cette perspective connaissent de nombreux déboires, et pour l’instant il ne semble pas évident que la prospection sur ces nouvelles terres donne plus de résultats que ceux obtenus sur les anciennes. On the importance of good deeds in Islam.

Objective To determine the attitudes of Egyptian patients regarding their participation in research and with the collection, storage and future use of blood samples for research purposes. Design Cross-sectional survey. Study population Adult Egyptian patients (n=600) at rural and urban hospitals and clinics. Results Less than half of the study population (44.3%) felt that informed consent forms should provide research participants the option to have their blood samples stored for future research. Of these participants, 39.9% thought that consent forms should (...) include the option that future research be restricted to the illness being studied. A slight majority (66.2%) would donate their samples for future genetic research. Respondents were more favourable towards having their blood samples exported to other Arab countries (62.0%) compared with countries in Europe (41.8%, p<0.001) and to the USA (37.2%, p<0.001). Conclusions This study shows that many individuals do not favour the donation of a blood sample for future research. Of those who do approve of such future research, many favour a consent model that includes an option restricting the future research to the illness being studied. Also, many Egyptians were hesitant to have their blood samples donated for genetic research or exported out of the Arab region to the USA and European countries. Further qualitative research should be performed to determine the underlying reasons for many of our results. (shrink)
